Eligibility and your role as an Assistance Dog handler
Please note eligibility for the D4LAD Program requires the following;
- Evidence that you, or the person you are filling out this form for, have a disability as defined in section 5 of the Guide, Hearing and Assistance Dogs ACT, 2009
- Evidence that an Assistance dog is required and would reduce the need for external supports
- Demonstrate that either you can control/handle the animal as a primary handler or you have an alternative or secondary handler that can assist with animal handling.
- Have a dog breed that is not considered a dangerous or restricted breed
- Have already desexed your dog or plan to do so (entire dogs do not qualify for the D4LAD Program)
- Canine health records including evidence of preventatives used, vaccination or immunity
- Willing to train using ethical methods and equipment that are approved by the organisation
- Prepared to undergo a public access test
- Prepared to comply with the Guide Hearing and Assistance Dogs ACT, 2009 and any organisational requirements outlined by Dogz 4 Life including appropriate follow up and re-assessment
